If a wrestler wins when he is under a great dog, one does not bother her one shot luck. I would call most of the fights you mentioned disorders rather than fins. Rahman KO Lewis was kind of fluky because it was a blow from nowhere that ended the fight. I doubt Rahman could land another blow like that when Lewis fought him 50 times. But it was no accident in a way because the boxing comes to finding a blow perfect grace and what Rahman did. Khan vs Barrera was not a fluke, because Barrera is cut and was not strong enough to stop the fight at first. Yes it was bad for four, but I've seen worse cuts to go the whole distance. Khan and still would love to have a rematch Barrera but was not a fluke by Khan was unfortunate for Barrera. A stroke of luck I can think of is Michael Sprott KO of Audley Harrison. It was basically looking at the floor and left alone again and connected flush on the chin and Harrison KO him. Harrison Sprott seems not to cause any problems at all, but everything you need in boxing is a hit. It was still kind of fluky though
